If not, please check to make sure you have answered all questions with a red asterisk, otherwise it won't submit and it won't pop up and tell you which questions were unanswered.
06-15-2019 - In the past 2 weeks we've been having a lot of problems with people not being able to submit this Questionnaire. When I went in and tested in today i discovered they've done a security process when you submit this. It comes up with the picture boxes asking you to choose all that have a sign, or a car, or a store front, or whatever before it will let you through. It wants to make sure you're not a robot. I had nothing to do with this screening, so I don't know how to undo it. So, if you have ANY problems at all, just email me. I have this Questionnaire in email form so all you have to do when I send it to you is click reply on the email and then click your cursor in the space below the question to fill it out. It's a lot better than going through the hassle trying to submit this form. So disappointing...
